We are happy to check on your benefits for you. Give us your full name, your date of birth, the name of the
insurance company, the 800 number on the back of the card, and the member or subscriber ID.
Ultimately it is your responsibility to contact your insurance company to verify your benefits. While the office staff is happy to obtain this information, we cannot guarantee the information to be accurate. Each insurance plan contracts with your place of employment indepedently, thus creating individual plans of service. There may be exclusions and restrictions placed on your coverage. Not all services are a covered benefit in all contracts.
Please remember that if you disagree with your insurance provider on matters of coverage, it is your responsibility to address the issues with them.
